| The
Trocar, Online Videojournal of Gynecologic and Surgical Endoscopy |
Is a paper intending to
fully exploit the Internet's resources in the field of scientific diffusion. It
is our opinion that an On-line Journal devoted to such a still expanding branch
of surgery as laparoscopy may "take advantage of" the Internet better than any
traditional paper. Thanks to the web resources, the published articles may
be furnished with photographs and videos which may better explain or point out
anatomical aspects, new technique and surgical procedures possibly described.
All the same, each article will be selected on the basis of the same scientific
strictness and thorough statistical analysis as traditional papers. |

| Role of Laparoscopic Tubal Recanalisation in modern era of ART
|
 |
First microsurgical reversal of the sterilization was done by Gomel & Winston in 1977.
Around 25-30% of the subfertile females have tubal damage and quite a few of them seek tubal micro-reconstruction for fertility enhancement, also those with tubal sterilization procedure seeking reversal for further child bearing are better candidates. This can now be achieved with principles of laparoscopic microsurgery.

| Extraperitoneal Laparoscopic Aortic Lynphadenectomy in Gynecological Cancer.

A fundamental principle in the assessing the prognosis of many gynaecological cancers such as cancer of the breast, cervix and endometrium is to have information about lymphatic invasion. These tumours spread predominantly by lymphatic channels...

| Laparoscopic Excision of Deep Fibrotic Endometriosis of the Cul-de-sac and Rectum
|

Diagnosis and treatment of endometriosis is the most frequent reason for gynecologic operative laparoscopy in the United States (Peterson et al,1990). Therefore, the laparoscopist must be thoroughly familiar with the current standards of diagnosis and management of this complex disease. The most common presentations of endometriosis include pelvic pain, infertility, and adnexal mass...

Power Doppler Sonographic Evaluation of the Vascularity of the Myomectomy
Comparison between Laparoscopic and Laparotomic Myomectomy.
|
 |
Study objective: To study times and conditions of the vascular repair of the uterine "scar" after laparoscopic and laparotomic myomectomy by means of transvaginal Color- Doppler ultrasonography. To check for defective healing along the uterine scar in the immediate post-operative period.

| Laparoscopic Management of Hypogastric Vein Injury during Pelvic Lymphadenectomy
|
 |
The first laparoscopic lymphadenectomy was reported in the late 1980’s and safety of this procedure has been largely proved.
Familiarity with instrumentation and management of complication is a prerequisite to perform this procedure.
In this case report we describe the successful management of a hypogastric vein injury during a pelvic laparoscopic lymphadenectomy.

| Laparoscopic rectosigmoid resection in case of deep endometriosis
|
 |
Intestinal endometriosis is a disabling disease present in 6% to 30% of deep endometriosis cases. Colorectal endometriosis requires surgical treatment that can be performed by abdominal route or by laparoscopy.
The present study describes the total laparoscopic rectosigmoid resection in case of deep endometriosis with bowel involvement.

|
 |
A case of unicornuate uterus with cavitary non-communicating rudimentary horn suddenly revealing with acute abdomen is presented. The patient was managed by laparoscopic miniaccess hemihysterectomy.

| The importance of Visual Port Insertion During Laparoscopy
|
 |
Patient safety is finally being institutionalized due to growing concern over the terrible cost of inadvertent human error. Medicine's punitive perfectibility model in dealing with unintended injury is slowly evolving to accept error during surgery, as an inevitable yet manageable reality of operations (Leap, 1994).

The development of surgery in the field of oncology is currently distinguished by the effort to decrease the aggressiveness and the morbidity of surgical treatment without lowering the chances of survival. The application of laparoscopy in the field of gynecologic oncology is not only a way to achieve these goals but also to improve the surgical staging of gynecologic malignancies.
